Investigators blamed on the airline employee “Yak Service” Vadim Timofeev

Main Investigation Department of the Investigative Committee of Russia (MRS) has completed a criminal investigation of the crash of the Yak-42, in killing a team of hockey club “Lokomotiv” almost at full strength. This was reported by the press service of the TFR.

The Yak-42 crashed September 7, 2011 in the village near Yaroslavl Yaroslavl. At takeoff the plane broke into several pieces. On board were 37 passengers and 8 crew members, including the players and staff of HC “Lokomotiv”. In accident survived two people – hockey player Alexander Galimov and crew member Alexander Sizov.

Criminal proceedings were instituted against the former deputy director general for the organization of flight operations Airlines’ Yak Service “Vadim Timofeev. He is accused of violating safety rules and operation of air transport. A criminal case has been referred for confirmation of the indictment and subsequent transfer to the court for consideration on the merits.

According to investigators, Timofeev responsible for the organization of flight operations, the maintenance of the qualification of flight personnel, enhancing their professional level, the organization of training and proficiency testing of pilots. In addition, it had the right to admission Timofeev crews to fly and right suspension in the event of inadequate qualifications noted in the TFR. “After reviewing all the facts and circumstances leading up to the tragedy, investigators determined that the crew was allowed Timofeev illegally, in violation of the rules of safe operation of air transport, and at the time of the disaster had no right to perform solo flight,” – said the agency.

The investigators found that the admission of the commander of the vessel has been issued on the basis of Timofeev falsified documents. In addition, the co-pilot at the time of the fatal flight did not graduate retraining in the Yak-42 and had no right to perform operations. Timofeev not control the passage of professional training pilots regularly withdrew them from the training sessions and illegally allowed to fly, it was reported. “That is Timofeev was known for certain about the failure to pass the flight personnel training and, consequently, the absence of the full amount of the crew skills for performing a safe flight,” – explains the TFR.

Also, the investigation revealed that Timofeev almost did not analyze the data flight recorders, fixed bugs killed the commander of the aircraft during the previous flight. “A bugs as it turned out, there were many, some of which could pose a real threat to flight safety,” – said in a statement. Timofeev took no action and he deliberately kept to allow for the flight of the commander. Moreover, the crew was formed in such a way that the two pilots and a flight engineer performed together only the third flight.

This is a part of the crew never practiced on the simulator action in case of emergency situation. And for the first time such a situation occurred precisely during the ill-fated flight on board the Yak-42, the investigators found. “So now we can say with certainty that it is the proper approach to the training of flight crews in this situation would help all the same to avoid the tragedy that claimed the lives of 44 people. Regarding the state of the aircraft, the results of examinations confirmed its full serviceability, “- noted in the Raman spectra.
